We are looking for someone who holds a high amount of empathy and compassion, and who can make a real difference - not only to our patients but to their family and support network.
The postholder will provide support to patients in their own homes or preferred place of care who have palliative or end of life care needs, raising clinical standards and promoting a learning environment for other health care providers.
They will be responsible for planning, assessing, implementing, and evaluating care in partnership with the Multidisciplinary Teams and out of hours providers.
A clean driving licence and use of a vehicle with business insurance is essential.
Part-time: 30 hours a week to be worked flexibly to include days, nights, weekends and bank holidays on a shift rota basis
Main duties of the job
The Senior Staff Nurse - Community role will contribute to the provision of specialist palliative and end of life care in the community, being based at Mountbatten Hampshire.
Under the supervision of the Locality Leads, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care, as well as providing advice and support to patients, their families and other healthcare professions within the community.
