We are seeking a compassionate and dedicated Community Nurse (Palliative Care) to join our team, providing high-quality, person-centred care to patients with life-limiting illnesses in their own homes.
This rewarding role involves supporting patients and their families through complex physical, emotional, and psychological needs, helping them achieve the best possible quality of life. You will work closely with multidisciplinary teams, including GPs, specialist nurses, therapists, and hospice services, to deliver coordinated and holistic care.

Main duties of the job
The successful candidate will work autonomously and as part of a team contributing to the provision of 24/7 Specialist Palliative/End of Life Care. They will be responsible for not only providing care, but for providing advice and support to patients, their families and other healthcare professions within the community.
The postholder will provide support to patients in their own homes or preferred place of care who have palliative or end of life care needs, raising clinical standards and promote a learning environment for other health care providers.
They will be responsible for planning, assessing, implementing, and evaluating care in partnership with the Multidisciplinary Teams and out of hours providers.
