The Role
We are looking for an organised, compassionate, and proactive Care Co-ordinator to join our team. This is a key role within the agency, responsible for ensuring the smooth day-to-day running of care services. You will work closely with care staff, clients, and their families to coordinate care packages that meet individual needs and regulatory standards.
Key Responsibilities
- Scheduling and allocating care workers to meet client needs efficiently.
- Acting as a first point of contact for clients, families, and staff, handling queries promptly and professionally.
- Participating in the on-call rota to provide out-of-hours support as required.
- Liaising with health and social care professionals to ensure a joined-up approach to client support.
- Providing hands on care support if required.
About You
- Previous experience in a care co-ordination or similar role within the health and social care sector preferred.
- Strong organisational and time-management skills, with the ability to prioritise and adapt in a fast-paced environment.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and staff.
- Competent IT skills.
- A flexible approach, including willingness to work shared on-call.
- A full UK driving licence and access to a vehicle preferred.
