An exciting opportunity has arisen to take on a rewarding role that truly makes a difference in the lives of people across our Island community. We are recruiting a Reablement Leader to join our compassionate and dedicated Community Reablement and Outreach team.

About the Role

As a Reablement Leader, you will:

  • Case manage individuals referred to the service, completing assessments, reviews, and care planning to ensure person-centred, high-quality support.
  • Lead a small team of support workers, providing supervision, guidance, and regular one-to-one and team meetings.
  • Maintain accurate records that reflect the needs and goals of the people we support, guiding staff to deliver care confidently and competently.
  • Deliver hands-on care when required, leading by example and ensuring best practice.

Key Responsibilities

  • Complete thorough referrals, assessments, and reviews—both written and electronic—always involving the person being supported.
  • Seek feedback from clients and staff to continuously improve service delivery.
  • Work collaboratively with health and social care professionals to achieve positive outcomes.
  • Conduct observations, competency assessments, and maintain detailed records of staff performance and development.
  • Support people transitioning from hospital to home or those needing extra help to regain independence.

What We’re Looking For

We’re seeking someone who:

  • Has experience working within relevant statutory frameworks.
  • Understands reablement principles—working with people, not for them.
  • Can build trusting relationships with empathy and sensitivity.
  • Is confident in motivating others and influencing positive change.
  • Has strong ICT skills and is comfortable using Microsoft applications.
  • Holds a full UK driving licence and has access to a vehicle with business use insurance.
