Support Worker & Complex Care Transition Team Members 

 

We are seeking health and social care support staff to join our Support Liaison team and our Transitions Team.

 

We are looking for positive, enthusiastic support staff who want to make a difference to the lives of people we support. We take a pro-active, person-centred approach to assist individuals to achieve their goals. Kindness will be at the core of everything you do, it touches upon every aspect of our specialist support.

Support Worker - We don’t mind if you have experience or not as a full induction and training is provided – all you need is a general interest and compassion for this role and excellent communication skills.

Transition Team members - you will be supporting people through emergency situations and will need to be able to work dynamically and flexibly to meet people’s identified outcomes. The role will require some transition work in other counties outside of Hampshire which will be fully funded and discussed with you as part of the transition support.

We care about all staff as individuals and appreciate that everyone brings with them different skills and experiences.

 

Requirements:

Excellent communication skills are essential to this role.

Drivers highly desirable.

Full and part time positions available.

Day and Night shifts available
