These are exciting times on the Isle of Wight within Adult Social Care, the whole service has supported a redesign that enhances strength-based practice, working with the community and our third sector partners.

In 2021 the team were finalists in the national MJ awards and LGC Care and Health Integration Awards and we were winners of the Isle of Wights NHS Quality Improvement and Innovation Award as part of Integrated Discharge Team (which had national recognition) and we also won the Isle of Wight Councils Staff Appreciation Award 2022 for Collaborative Team Effort!

We now have a Social Work Assistant vacancy in our Wellbeing & Access Hub, supporting people at first point of contact.

This is an exciting opportunity, working alongside Crisis Rapid Response (our health colleagues), to reduce admission to hospital and reduce the need for long term care. We do this by signposting to services and arranging support via teams such as reablement services and outreach, to help each person regarding their independence

You’ll work as part of the first point of contact team for Adult Social Care, taking referrals by telephone, providing information and signposting.

You’ll be responsible for timely assessments that are strength based, support with planning that promote and maintain independence, choice and autonomy of people.

You’ll use your skills to implement and promote departmental safeguarding procedures and adhere to departmental recording policy as well as attend multi-agency discharge planning meetings to ensure timely discharges.

You’ll also support carers and family members sharing and advising where required as well as offering Carers Assessments in line with the Care Act 2014.

The successful candidate will be suitably qualified with an NVQ3 in Care or equivalent qualification or demonstrable experience in the care sector (working with vulnerable adults in a senior position within a registered provider setting or day care) and willingness to undertake the NVQ level 3 in Care should you not hold it.

You will be able to demonstrate knowledge and understanding of The Care Act 2014, Mental Capacity Act 2005, Data Protection Act 1998 and evidence of ability to work to practice guidance/procedure.
