Hours: This role is 30 hours a week initially- with the potential to go to 40 hours a week as service develops. Requirement to be part of the on-call team.

Location: Landguard Manor, Shanklin, IOW.

Salary: £24,780.

Interviews will take place Thursday 2nd May.

Service user involvement: Currently we have a group of individuals who would require being seen some weekly and some fortnightly.

Care Planning and overseeing the care management of our Service users with complex mental health needs.

Identifying and working with service user with complex needs around coping strategies when in crisis, and their staffing team to develop the confidence and ability of the staffing team to manage risk taking behaviours of service user.

Meeting weekly with the complex mental health service users for an hour each as a talk time session to look at how the week has been for them, challenges faced, goal setting and pathway planning alongside the site lead at each provision.

Develop and expand care plans, risk assessments and crisis intervention plans, assigning actions to site leads, with robust action plans that rag rate risk and reflect a journey of recovery focus, liaising with professionals involved.

The Provisions.

Manage and oversee the site leads of our supported living provisions whereby mental health is the primary need and offer the overarching supervision of their practice.

Oversee and manage the data reporting from each individual with complex mental health needs and report to external partner agencies if necessary.

Support site leads in signposting and access services within the community for the needs of service users you oversee.

 

Safeguarding and MDT working:

Work alongside Call on Me designated safeguarding officer to ensure safeguarding’s are managed and reported in a timely manner.

Attend MDT meetings and safeguarding meetings to advocate for the service users on your case load.

Call On Me Development of services

Work alongside the Registered Manager in the development and expansion of the mental health supported living provisions, looking at and being involved in the development of the provisions and services that the provision will offer and the assessment process for service users to be considered for the provision.

Have direct line management responsibility for the Wellbeing and Community Coordinator and support them to develop and implementing staff wellbeing initiatives, training and a positive culture within Call on Me.

Implement and develop staff training events around Mental Health.

Develop and implement wellbeing resources, videos and strategies that are shared with site leads and implemented into the day to day life’s of the individuals we support.

Work alongside the senior leadership team and be involved in the planning and direction of travel for Call on Me as it continues to grow.

Create and develop a young adult’s target group and work alongside them in developing new initiatives for our young people within call on me and the development of provisions.

Create and develop alongside representatives from the individuals we support the values/ visons of Call on Me and implement these across the service.

To understand and advocate for the challenges being faced by our service users and working as part of the leadership team to look at new initiatives and ways in which we can develop Call On Me to support the changing needs of the individuals we support.
